Job Description
- Assist in administrative work in the delivery of advisory services to patients and their families.
- Conduct assessment / home visits when necessary.
- Liaise with relevant agencies, authority, and patients’ families.
- Assure that all referrals are attended to by relevant personnel.
- Liaise with nurses on patient-related matters.
- Act as a channel of communication between management and the customer care coordinator.
- Handle difficult customers and communicate escalated issues to the Manager.
- Keep updated records of patients (electronic or written data).
- Stay up-to-date with relevant information on government policies regarding elderly care services and funding.
